Insurances
The procedures we perform are covered by most insurance plans. Your coverage depends on the details of your policy. You will be responsible for any deductible, co-pay or non-covered service according to your plan.
Our office will be pleased to work with you and your insurance company to minimize any issues that might arise. Our office will also file claims to any insurance plans you have. If anesthesia services are required, charges for these services will be billed separately.
To prevent misunderstandings, we will verify your benefits and review them with you prior to your visit with us. Please provide full insurance information in advance. You can mail, email, or call this information into our office. This will assist us in obtaining insurance approval as quickly as possible on your behalf.
For patients without insurance, or if your deductible has not yet been met, payment in full is required at the time of the first visit. For your convenience, we accept VISA, MasterCard and American Express.
Appointments
Please call either of our offices during regular office hours – Monday through Friday from 8AM to 4:30PM and indicate your preferred location. When coming for your FIRST appointment, make sure to bring a list of your present or recent medications and the names of any physicians you are seeing regarding your present condition. It is imperative to bring all recent tests with you. This includes laboratory studies and chest x-rays.
Prescriptions
If you need a prescription renewed, call us during office hours. You can expect a 24 hour turnaround time. By law, we are not allowed to call in renewals for painkillers to your pharmacy. If you are on painkillers and in need of a refill, plan to give us a renewal notice seven days in advance, as you will need a written prescription. No Exceptions.
Emergencies
If you have an emergency, during regular business hours, call our office immediately. If our office is closed, call our emergency service toll free at 1-877-405-5088 and provide the service with your doctor’s name. You can also call your family doctor, dial 911 or go the emergency room of your local hospital.
